Op onze unieke PG-plus afdeling De Vlindertuin (onderdeel van locatie Sonnevanck, prachtig midden in de bossen tussen Ermelo en Harderwijk) zoeken wij een doortastende coördinerend verzorgende IG of EVV'er. Lees de spelregels van de startbonus Als coördinerend verzorgende IG/ EVV'er ben jij de spil en de regisseur van het zorgproces. Je zorgt dat de dagelijkse zorg staat als een huis: - De rode draad bewaken: Je stelt zorgleefplannen op. Voor deze coördinerende taken krijg je extra uren toebedeeld. - Verbinden: Je leidt multidisciplinaire overleggen en bent het vertrouwde aanspreekpunt voor collega's, familie en behandelaren. - Kwaliteit & Actie: Je pikt subtiele veranderingen bij bewoners direct op en onderneemt actie. - Warmte & Ondersteuning: Je stimuleert de zelfredzaamheid en biedt extra, liefdevolle begeleiding in complexe situaties (zoals in de terminale fase). Waar ga je werken? Je werkt op De Vlindertuin, een unieke PG-plus afdeling op locatie Sonnevanck, midden in de bossen tussen Ermelo en Harderwijk. Hier bieden we een prikkelarme omgeving in geschakelde woningen voor bewoners met dementie en soms onbegrepen gedrag. Denk aan onrust, dwalen, roepen of verbale en fysieke ontremming.
