Wij ondersteunen mensen met dementie en geheugenproblemen die thuis wonen. Daarbij draait het niet alleen om goede zorg, maar ook om een fijne dag, zoveel mogelijk eigen regie en aandacht voor de mensen om onze cliënten heen. Wat ga jij doen? Als Verzorgende IG of Begeleider MZ4 combineer je persoonlijke zorg met begeleiding en welzijn. Je helpt bij de dagelijkse verzorging en voert verpleegtechnische handelingen uit als dat nodig is. Maar je kijkt ook verder: wat geeft iemand plezier, wat lukt nog zelfstandig en hoe houden we het dagelijks leven vertrouwd? Soms zit het verschil in iets kleins: samen koken, een wandeling maken of helpen om structuur in de dag te brengen. We bieden ook ruimte voor extra welzijnsmomenten, vaak in de middag. Dat willen we in de toekomst verder uitbreiden. Je hebt bovendien oog voor de partner en mantelzorger. Je luistert, signaleert wanneer de zorg zwaarder wordt en denkt mee over wat nodig is om het thuis samen vol te houden. Daarbij werk je samen met collega's, de Zorgtrajectbegeleider Dementie en andere betrokken professionals. Daarnaast houd je het cliëntdossier actueel, voer je evaluatiegesprekken en neem je deel aan multidisciplinair overleg. Zo zorgen we er samen voor dat de zorg goed aansluit bij de behoeften van de cliënt. Eén team, samen rondom de cliënt Wij zijn een team van acht collega's in Wijchen Noord. We werken zelfstandig bij cliënten thuis, maar staan er nooit alleen voor. We delen onze kennis, bespreken wat we tegenkomen en zoeken samen naar passende oplossingen.
