Als Contactverzorgende ben jij het eerste aanspreekpunt voor onze bewoners en hun naasten. Je bent verantwoordelijk voor de coördinatie van de zorg en werkt samen met collega's om onze cliënten de best mogelijke ondersteuning te bieden. Je taken bestaan uit: ✓ Het opstellen, evalueren en bijstellen van zorgplannen ✓ Het ondersteunen bij dagelijkse verzorging, zoals wassen, aankleden en medicatie ✓ Het signaleren van veranderingen in de gezondheid of situatie van cliënten ✓ Het onderhouden van contact met familie, mantelzorgers en andere zorgverleners ✓ Bijdragen aan een warme en veilige omgeving voor onze cliënten.
