Werken bij de Westerkim Verzorgende IG met coördinerende taken In een nieuw, duurzaam huis en in de Westerheem (geclusterd VPT) biedt de Westerkim welzijn en zorg aan onze bewoners. Door bewoners met een soortgelijke zorgvraag bij elkaar te laten wonen, kunnen we nog meer en beter aansluiten bij en focussen op hun welbevinden en zorgbehoefte . Als verzorgende IG met coördinerende taken ondersteun je onze bewoners om ondanks hun lichamelijke beperking, ouderdom of dementie een goed leven te leiden. Daarbij kijken we juist naar wat onze bewoners wel kunnen in plaats van uit te gaan van wat niet meer kan. Je fungeert als eerste aanspreekpunt van onze bewoners en hun naasten. Je stelt de zorgbehoefte vast, stelt het zorgplan op, evalueert- en stelt deze bij. Ook coördineer je het zorgproces (zoals het MDO), signaleer je afwijkingen en stel je bij waar nodig. Daarnaast instrueer je je collega zorgmedewerker bij het uitvoeren van het zorgplan. Wie ben jij? - je bent een professional met een diploma MBO verzorgende IG niveau 3 en je hebt de ambitie om je te ontwikkelen in je vak. - je hebt ervaring in de ouderenzorg en hebt affiniteit met de doelgroep. - je bent flexibel en bereid alle voorkomende diensten te werken. - je kunt zelfstandig werken, maar ook goed functioneren in een team. - je bent geduldig, hebt een goed empathisch vermogen en beschikt over goede communicatieve vaardigheden. - je hebt een open en respectvolle houding en kunt samenwerken met mantelzorgers, familie, vrijwilligers en andere disciplines en bent in staat feedback te geven en te ontvangen. - je bent in staat om accuraat verslag te leggen, te overleggen en over te dragen.
