Jij zorgt voor onze cliënten in de wijk en in de aanleunwoningen in zowel Hasselt als Zwartsluis. Je verricht natuurlijk de verpleegtechnische handelingen maar je denkt ook mee hoe dingen anders of beter kunnen. Ons team bestaat uit VIG-ers en verpleegkundigen. Je werkt zelfstandig op je route. We hebben binnen ons team korte lijnen en als het nodig is kun je terugvallen op een ervaren collega. Zo is er iedere werkdag een wijkverpleegkundige aanwezig waar je altijd terecht kunt. Wij zijn een stabiel team waar een hele goede sfeer heerst en we zorgen samen dat je goed wordt ingewerkt! Waar ga je werken? Wij hebben 1 combi team voor de locaties Hasselt en Zwartsluis. Het kan zijn dat je af en toe ingezet wordt in Genemuiden. Er is een dienstauto en elektrische fiets beschikbaar in overleg met collega's. Een eigen auto is daarom wel nodig. Als wijkverpleging binnen IJsselheem zijn wij voortdurend in ontwikkeling. Omdat mensen steeds langer thuis blijven wonen, heeft de wijk de toekomst! We streven ernaar om zo lang mogelijke routes op te zetten voor een goed rooster. Onze ochtendroutes starten om 07:00 uur en eindigen rond 12:00 uur (4 routes). Daarnaast is er op bepaalde dagen een mamaroute van 08:45 tot 11:45 uur. De dagdiensten variëren, maar duren uiterlijk tot 15:30 uur. Verder is er één avonddienst van 15:30 tot 23:00 uur.
