Wil jij mensen met een ondersteuningsvraag begeleiden naar een betekenisvolle werkdag? Krijg je energie van het coachen van cliënten op de werkvloer en werk je graag in een afwisselende omgeving waar je écht het verschil maakt? Dan is deze functie iets voor jou. Wat ga je doen? Als Werkbegeleider begeleid je cliënten tijdens hun dagelijkse werkzaamheden binnen Pameijer Werkt West. Je ondersteunt mensen met uiteenlopende ondersteuningsvragen bij het ontwikkelen van werknemersvaardigheden, het vergroten van hun zelfstandigheid en het behalen van persoonlijke doelen. Daarbij kijk je altijd naar wat iemand wél kan en stimuleer je cliënten om zoveel mogelijk zelf de regie te nemen. Je begeleidt cliënten op de werkvloer en zorgt voor een veilige, gestructureerde en prettige werkomgeving waarin zij zich kunnen ontwikkelen. Je biedt ondersteuning waar nodig, signaleert veranderingen in gedrag of ondersteuningsvragen en speelt hier op een passende manier op in. Je werkt methodisch met ondersteuningsplannen, rapporteert in het elektronisch cliëntendossier (ECD) en onderhoudt contact met collega's, verwanten en andere betrokkenen. Samen met je team zorg je ervoor dat iedere cliënt de begeleiding krijgt die past bij zijn of haar mogelijkheden. Geen werkdag is hetzelfde. Je schakelt gemakkelijk tussen verschillende werkzaamheden, begeleidt diverse cliënten en werkt op meerdere locaties binnen Rotterdam-West. Hierdoor is flexibiliteit belangrijk en krijg je veel afwisseling in je werk. Als Werkbegeleider: - Begeleid je cliënten tijdens hun dagelijkse werkzaamheden. - Stimuleer je zelfstandigheid, ontwikkeling en werknemersvaardigheden. - Werk je met ondersteuningsplannen en rapporteer je in het ECD. - Signaleer je veranderingen in gedrag of ondersteuningsvragen en speel je hierop in. ...
