Jij helpt hen om weer grip te krijgen op hun geldzaken en voorkomt in samenspraak dat problemen verder oplopen. Jij ondersteunt inwoners bij het creëren van overzicht in hun financiën, denkt mee over passende betalingsregelingen en wijst hen op regelingen en voorzieningen waar zij gebruik van kunnen maken. Ook bied jij zo nodig hulp bij het aanvragen van toeslagen en andere regelingen. De maandelijkse signalen van vaste lasten partners zijn de basis om tot actie over te gaan; jij brengt de situatie in kaart en neemt het initiatief om met inwoners in contact te komen (per brief, e-mail, telefonisch of soms een huisbezoek). Ook onderhoud jij contact met de vaste lasten partners, zodat de lijnen kort blijven en de samenwerking soepel verloopt. Wanneer nodig, verwijs jij inwoners door naar het wijkteam van TOM in de buurt, de helpdesk geldzaken, schuldhulpverlening of andere passende ondersteuning. Als Wijkcoach begeleid je inwoners, als zij behoefte hebben aan ondersteuning bij bijvoorbeeld zelfstandig thuis blijven wonen, dagbesteding en thema's als rouwverwerking, administratie, relatie en scheiding, omgaan met stress en huiselijk geweld. Je biedt de inwoner een luisterend oor, een reikende hand én je stimuleert de inwoner ook om zelf, of met hulp vanuit...
