Vacature details Solliciteren Als Wijkcoach begeleid jij inwoners, zodra zij behoefte hebben aan ondersteuning over bij zelfstandig thuis blijven wonen, dagbesteding en thema's als rouwverwerking, administratie, relatie en scheiding. Je biedt de inwoner een luisterend oor, een reikende hand én je stimuleert de inwoner ook om zelf te doen, wat mogelijk is. Een greep uit jouw dag Als een inwoner contact heeft opgenomen met Tom in de buurt, volgt er een gesprek met de host. Vervolgens wordt er een leefgesprek gevoerd met de casemanager. In dat leefgesprek worden de ondersteuningsdoelen van de inwoner besproken. Na het leefgesprek start jouw traject. Als Wijkcoach (hbo) behandel je casussen met diverse problematiek. Bijvoorbeeld bij de aanwezigheid van meervoudige problematiek, of bij de aanwezigheid van bijzondere omstandigheden of opvallend gedrag van de klant. Dit vraagt vaak om meer theoretische kennis en een meer kritische, onderzoekende houding, zelfstandige analyses en beoordeling. De uitvoering doe jij aan de hand van plan-do-check-act. Werkinhoudelijk ben je ook een vraagbaak en sparringpartner voor wijkcoaches mbo. Je biedt en initieert de benodigde hulp en ondersteuning aan de inwoner, maar stimuleert ook dat de inwoner zelf doet, wat mogelijk is. Het doel is: de inwoner in zijn eigen kracht zetten. Je luistert, onderzoekt behoeften en eigen mogelijkheden, denkt mee, geeft inzicht en advies, fac...
