Dan ben jij de wijkverpleegkundige die perfect past in ons betrokken team van Lelie zorggroep Lage Land en Ommoord. Als wijkverpleegkundige ben jij de spil in het zorgnetwerk van onze cliënten. Je stemt de zorg af op ieders persoonlijke situatie, coacht collega's en mantelzorgers, indiceert de zorg en hebt oog voor preventie. Je werkt zelfstandig, maar staat er nooit alleen voor: we doen het samen. Ons deskundige team biedt zorg aan een diverse groep cliënten in de wijken Lage Land en Ommoord, van kwetsbare ouderen tot cliënten die tijdelijk ondersteuning nodig hebben na een ziekenhuisopname. We bieden je een warme en betrouwbare werkomgeving waarin collegialiteit, eigen initiatief en professionele ontwikkeling centraal staan. Zo kun jij met vertrouwen en plezier doen waar je goed in bent: samen zorgen voor de beste zorg in Lage Land en Ommoord. Dit vragen wij van jou - Je hebt een afgeronde opleiding Verpleegkunde op hbo-niveau 5 en bent BIG-geregistreerd. - Je hebt bij voorkeur ervaring in de wijkverpleging. - Je kunt goed zelfstandig werken en houdt van samenwerken in een hecht team. - Je bent enthousiast, zorgvuldig, flexibel en communicatief vaardig. - Je beschikt over eigen vervoer. - Wij vinden het belangrijk dat je onze missie en visie onderschrijft, jij herkent of respecteert de christelijke identiteit van onze organisatie.
