Voor een tijdelijke vervanging tijdens zwangerschaps‑ en bevallingsverlof zoeken wij een enthousiaste collega in regio Stiens! Wat ga je doen? Als wijkverpleegkundige vervul je een centrale rol in de zorgverlening in jouw wijk. Je hebt regie over het zorgproces en weet wat er nodig is om cliënten passende ondersteuning te bieden. Je stelt indicaties en zorgplannen op en bewaakt de uitvoering daarvan. Ook in complexe situaties organiseer en coördineer je de zorg en behoud je het overzicht. Daarbij geef je vakinhoudelijke aansturing aan collega‑verpleegkundigen en verzorgenden IG. Je werkt intensief samen met huisartsen, mantelzorgers en andere ketenpartners en draagt bij aan goede afstemming in de wijk. Vanuit jouw deskundigheid stimuleer je cliënten om eigen regie te behouden en keuzes te maken, met positieve gezondheid als uitgangspunt. Daarnaast lever je een actieve bijdrage aan de ontwikkeling van zorgbeleid en de deskundigheid binnen het team. Zo draag je eraan bij dat cliënten zo lang mogelijk zelfstandig thuis kunnen blijven wonen. Wie zijn wij? Wij zijn Thuiszorg Het Friese Land.
