Dat is: Jij en Woonzorg Flevoland. Als HBO-verpleegkundige niveau 6, ben jij de spil in het zorgproces van onze cliënten. Jij neemt de eindverantwoordelijkheid voor het beoordelen van de zorgbehoeften en het coördineren van de totale zorg. Je hebt een coachende rol waarbij je de verpleegkundigen niveau 4 begeleidt en ondersteunt in hun beroepsuitoefening. Je bent in verbinding met de verpleegkundigen en draag bij aan de deskundigheidsbevordering van het team. Je bent voortdurend bezig met de doorontwikkeling van onze wijkverpleging in Almere-Haven (team-oost), waarbij kwaliteit, professionalisering, verbinding en samenwerking met andere (zorg)partners centraal staan. Ook participeer je actief in projecten en werkgroepen om bij te dragen aan innovatie en beleidsontwikkeling binnen de wijkverpleging Je werkt vanuit onze locatie Leemwierde in Almere Haven, waar je tijdens kantoortijden 8-uurs diensten werkt. Je gaat op intake bij nieuwe cliënten, draagt zorg voor dossiervorming (één keer per week op route) en je hebt te maken met langdurige zorg en Volledig Pakket Thuis (VPT). Daarnaast werk je nauw samen met verschillende disciplines op deze locatie om de best mogelijke zorg te bieden. Zo zorgen wij voor jou: - Een salaris conform de cao VVT FWG 50-55; tussen de € 3.644,59 en € 5.230,32 per maand op basis van 36 uur per week. Inschaling is op basis van ervaring en kennis. - Goede secundaire arbeidsvoorwaarden zoals: 8% vakantiegeld, eindejaarsuitkering van 8,33% en een reiskostenruilregeling; - Een contract voor 24 - 32 uur, met de mogelijkheid tot direct een contract voor onbepaalde tijd. - Er staat een dienstauto voor je klaar voor de werkzaamheden in de wijk. - Ruimte om je verder te o...
