Je kijkt verder dan alleen de zorgvraag van vandaag en hebt oog voor de persoonlijke situatie, gezondheid en zelfredzaamheid van iedere cliënt. Met jouw kennis en klinische blik weet je de zorgbehoefte goed in kaart te brengen en passende zorg te organiseren. Je werkdag is afwisselend en biedt veel ruimte voor eigen verantwoordelijkheid. Je bezoekt cliënten thuis, beoordeelt hun gezondheidssituatie en stelt vast welke verpleegkundige zorg nodig is. Hierbij voer je indicatiegesprekken, stel je zorgplannen op en evalueer je of de ingezette zorg nog aansluit bij de behoeften van de cliënt. Daarnaast houd je veranderingen in de gezondheid van cliënten nauwlettend in de gaten. Je signaleert risico's, denkt vooruit en schakelt waar nodig andere zorgprofessionals in. Je werkt nauw samen met onder andere huisartsen, verzorgenden IG, verpleegkundigen, fysiotherapeuten en mantelzorgers om de zorg zo goed mogelijk op elkaar af te stemmen. Ook speel je een belangrijke rol in de begeleiding en ondersteuning van je collega's. Je denkt mee over complexe zorgvragen, deelt je kennis en draagt bij aan de kwaliteit van zorg binnen het team. Uiteraard horen het bijhouden van cliëntdossiers, rapporteren en het coördineren van de dagelijkse zorg ook bij jouw werkzaamheden. Kortom: als wijkverpleegkundige combineer je verpleegkundige expertise met coördinatie, klinisch redeneren en persoonlijk contact. Je krijgt de ruimte om zelfstandig beslissingen te nemen en maakt iedere dag het verschil voor mensen die zo prettig en zelfstandig mogeli...
