Samen kijken we naar wat cliënten zelf nog kunnen en zetten we waar mogelijk slimme zorgtechnologie in om hun zelfstandigheid en eigen regie te versterken. Jouw functie Als wijkverpleegkundige ben je verantwoordelijk voor het indiceren, organiseren en coördineren van zorg in de thuissituatie. Je kijkt verder dan de zorgvraag alleen en hebt oog voor de mens achter de cliënt. Daarbij stimuleer je cliënten om zoveel mogelijk zelf te blijven doen en ondersteun je hen waar nodig. Je maakt deel uit van wijkteam Assen en hebt een belangrijke rol in het waarborgen en verder ontwikkelen van de kwaliteit van zorg. Je werkt nauw samen met collega's wijkverpleegkundigen uit andere regio's, mantelzorgers en andere zorgprofessionals en weet verbinding te maken tussen alle betrokkenen rondom de cliënt. Naast je coördinerende taken draai je mee in de zorgverlening en ben je waar nodig inzetbaar in route en daarbij rij je in een auto van Zorggroep Drenthe. Dit neem je mee - Een afgeronde hbo-opleiding Verpleegkunde en een geldige BIG-registratie; - Ervaring met en kennis van indiceren volgens het normenkader (pré); - Affiniteit met de ouderenzorg; - Goede communicatieve en organisatorische vaardigheden; - Een zelfstandige werkhouding en een proactieve instelling; - Rijbewijs B.
