Kom daarom ons team versterken als enthousiaste en positief ingestelde: Wijkverpleegkundige in de thuiszorg 32 - 36 uur per week Dit ga je met veel plezier doen. Als wijkverpleegkundige in de thuiszorg ben jij het gezicht in de wijk én de verbindende schakel binnen het zorgnetwerk. Samen met je cliënt bepaal je wat nodig is om hun zelfredzaamheid, eigen regie en kwaliteit van leven te bevorderen. Je stemt af met naasten en het netwerk en kijkt naar de mogelijkheden om (technologische) hulpmiddelen in te zetten die het verschil maken. Je werkt nauw samen met het thuiszorgteam, paramedici, welzijnsorganisaties, het sociaal domein en andere zorgprofessionals. Samen zorg je ervoor dat je cliënt (langer) thuis kan blijven in hun vertrouwde omgeving. Je bent het aanspreekpunt voor je team als het gaat om de kwaliteit van zorgverlening en hebt een adviserende en coachende rol richting je collega's. Innovatie staat bij ons hoog op de agenda. Zo zijn we koploper in het project "Langer Vitaal Thuis" (reablement), een interdisciplinair programma dat de zelfredzaamheid en kwaliteit van leven van cliënten versterkt. Dit uitdagende project, gekoppeld aan wetenschappelijk onderzoek, helpt cliënten om in hun eigen kracht te staan, gewoon thuis. Draag je graag bij aan dit soort innovaties? Mooi, want bij ons krijg je alle vrijheid om hierin je steentje bij te dragen! Je maakt deel uit van de vakgroep wijkverpleging binnen Samen.
