Buurtzorg Hulst zoekt een wijkverpleegkundige niveau 4, 5 of 6 Standplaats: Sint Jansteen - Opleidingsniveau: HBO Buurtzorg levert verpleging en verzorging thuis vanuit kleine, zelfsturende teams bestaande uit maximaal 12 (wijk)verpleegkundigen en wijkziekenverzorgenden. Met een gezonde dosis nieuwsgierigheid kun jij samen met de cliënt diens wensen en behoeften in kaart brengen, je weet de juiste prioriteiten te stellen en stemt daar de ondersteuning die je biedt op af. Preventie is een vanzelfsprekend onderdeel van de zorgverlening. Een wijkziekenverzorgende / ( wijk)verpleegkundige bij Buurtzorg streeft naar het leveren van 'de beste zorg thuis'. Je bent zichtbaar in de buurt en zoekt samen met je cliënten naar oplossingen die het welzijn bevorderen. Het uitgangspunt is de zelfredzaamheid van de cliënt te vergroten. Je verricht alle noodzakelijke begeleidende, verzorgende en verpleegkundige handelingen in het verpleegkundig proces. Je bent verantwoordelijk voor de intake, de zorgverlening en de eindevaluatie. Je werkt daarbij samen met collega (wijk)verpleegkundigen en wijkziekenverzorgenden. Je zorgt voor korte lijnen naar huisartsen, andere eerste en tweedelijns zorgverleners en naar informele zorg- en welzijnsorganisaties. Je bent een gediplomeerde wijkziekenverzorgende (niveau 3IG) / verpleegkundige in de wijk (niveau 4) / wijkverpleegkundige (HBO-V / MGZ, niveau 5) Salariëring vindt plaats conform de CAO VVT. Afhankelijk van je ervaring bepalen we de hoogte van jouw salaris. We stemmen het aantal uren dat je wilt werken graag met je af.
