Je indiceert en organiseert passende zorg voor cliënten in hun eigen vertrouwde omgeving en werkt ongeveer de helft van jouw contracturen zelf mee in de wijk. Daarnaast heb je een overstijgende rol binnen het wijkteam. Je bent een belangrijk aanspreekpunt voor collega's, coacht medewerkers, begeleidt leerlingen en draagt bij aan een goede planning en roostering. Samen met de manager zorg je ervoor dat het team optimaal kan functioneren en de kwaliteit en continuïteit van zorg gewaarborgd blijven. Je werkt voornamelijk van maandag tot en met vrijdag tijdens kantooruren. Incidenteel kan het nodig zijn om in de avond of het weekend te werken. Daarmee biedt deze functie een mooie combinatie van werken in de wijk, verantwoordelijkheid en meer regelmaat in je werkweek.
