Dan zijn wij op zoek naar jou! Functie Als Wijkverpleegkundige bij HappyZorg Heerenveen ben jij de spil in de zorg aan huis. Je indiceert, coördineert en verleent verpleegkundige zorg aan cliënten in hun eigen vertrouwde omgeving. Je kijkt verder dan alleen de zorgvraag en stimuleert cliënten om zo zelfstandig mogelijk te blijven. Je bent werkzaam in een klein team, waarbij je naast wijkverpleegkundige taken ook meewerkt in de routes. Je werkzaamheden bestaan onder andere uit: Stellen van verpleegkundige indicaties volgens de geldende richtlijnen. Coördineren en bewaken van de kwaliteit en continuïteit van zorg. Verlenen van zorg. Opstellen, evalueren en bijstellen van zorgplannen. Meewerken aan sturing en coaching van het team. Samenwerken met huisartsen, ziekenhuizen, mantelzorgers en andere zorgprofessionals. Signaleren van veranderingen in de gezondheidssituatie van cliënten en hier adequaat op inspelen. Over het bedrijf Bij HappyZorg geloven we dat goede zorg begint met oprechte aandacht. Wij vragen Jij bent een betrokken zorgprofessional die zelfstandig beslissingen durft te nemen en graag samenwerkt. Daarnaast beschik je over: Een afgeronde hbo-opleiding Verpleegkunde; Een geldige BIG-registratie; Goede communicatieve en organisatorische vaardigheden; Een zelfstandige, flexibele en cliëntgerichte werkhouding; Een rijbewijs B en de beschikking over eigen vervoer (indien van toepassing).
