De gemeente werkt voor inwoners van 26 dorpen en houdt bij haar plannen rekening met hun wensen, behoeften en mogelijkheden binnen de geldende wet- en regelgeving. Functieomschrijving Als WMO Consulent zorg je ervoor dat inwoners de ondersteuning krijgen die past bij hun situatie. Je hebt oog voor de balans tussen ondersteuning en zorg, veiligheid en leefbaarheid. Daarbij zoek je actief de verbinding met inwoners, collega's en andere betrokken partijen om samen tot duurzame oplossingen te komen. Je onderzoekt wat een inwoner zelf kan doen en welke ondersteuning vanuit de gemeente nodig is. Daarbij kijk je niet alleen naar de directe hulpvraag, maar ook naar de bredere thuissituatie. Waar nodig organiseer je passende hulp en ondersteuning en blijf je betrokken bij de ingezette oplossing. Wat ga je doen? - Je bespreekt en verdeelt samen met je collega's de nieuw binnengekomen meldingen. - Bij het oppakken van meldingen betrek je bijvoorbeeld collega's van schuldhulpverlening, jeugdhulp of de sociale basis. - Je overlegt met inwoners en betrokken professionals over wat nodig is om de thuissituatie goed te houden. - Je stelt samen met inwoners en betrokken partijen een plan met duidelijke doelen op. - Je maakt afspraken over wie wat doet en hoe je op de hoogte wordt gehouden. - Je legt huisbezoeken af om hulpvragen te bespreken en te bepalen wat inwoners nodig hebben om goed thuis te kunnen blijven wonen. - Je schrijft gemaakte afspraken uit in een plan en maakt de verdere administratie in orde. Verantwoordelijkheden Als WMO Consulent heb je een verantwoordelijke functie waarin een sociaal karakter en analytische vaardigheden samenkomen. De gemeente zoekt specifiek iemand die het werk ook in complexe leefsituaties goed kan uitvoeren en aansturen....
