Dit maakt ons de op één na grootste gemeentelijke organisatie in Drenthe. Daarnaast zijn wij een unieke organisatie door het werken voor 2 heel verschillende gemeenten. We zijn centraal gelegen en goed bereikbaar, zowel met de auto als het openbaar vervoer. Kijk voor meer informatie over onze organisatie op www.werkenbijdeswo.nl Opdracht Als Wmo consulent draag je actief bij aan de uitvoering van de Wmo. Je voert keukentafelgesprekken met cliënten en maakt naar aanleiding van de gesprekken rapportages en gespreksverslagen. Waar nodig stel je een indicatie, of je verwijst actief door naar voorzieningen in het voorliggende veld. Je overlegt proactief met partijen als maatschappelijk werk, zorgaanbieders, wijkverpleegkundigen. Je signaleert en meldt knelpunten, ook als deze niet Wmo-gerelateerd zijn, en bespreekt deze integraal met je collega's in het sociaal domein. Je denkt intern voortdurend actief mee over de ontwikkelingen binnen je eigen team maar ook over de ontwikkelingen binnen het sociaal domein. De werkzaamheden maken integraal onderdeel uit van de reguliere bedrijfsvoering en worden verricht binnen de bestaande organisatiestructuur. De medewerker maakt deel uit van het team, neemt deel aan interne overleggen en werkt onder dagelijkse aansturing van de verantwoordelijke manager.
