Je brengt nauwkeurig de situatie in kaart en weegt alle belangen mee. Je gaat hier deskundig en zorgzaam mee om. Vanuit meerdere perspectieven kom je tot een passende dienst- of hulpverlening, die zich richt op het vergroten van de deelname aan het maatschappelijk verkeer en de zelfredzaamheid van onze inwoners. Je bent goed op de hoogte van de sociale kaart en de voorliggende voorzieningen en weet deze op de juiste wijze in te zetten. Takenpakket: onderzoeken in welke mate mensen op eigen kracht -met hulp van het sociaal netwerk - algemeen (gebruikelijke) of wettelijke voorliggende voorzieningen in hun hulpvraag kunnen voorzien. De meldingen betreffen met name begeleiding en dagbesteding. de zorgvraag en behoefte inventariseren tijdens een (keukentafel)gesprek met de client • intensief samenwerken met collega's van het wijkteam 0-100 en externe partners om de integrale dienstverlening zo efficient mogelijk te laten verlopen. zelfstandig het hele proces van intake tot en met beschikking uitvoeren. als regisseur ben je aanspreekpunt bij wijzigingen in de situatie van de client. de inzet van hulp en voorzieningen periodiek heronderzoeken. eisen hbo bachelor Aantoonbare ervaring van minimaal drie jaar als wmo consulent Hans Houweling Algemeen directeur T: 010-4140282
