- Wolvega - 36 uur - HBO - € 3.568,- / € 5.096,- per maand Ervaren Wmo-consulent Voor een gemeente zijn wij op zoek naar een ervaren Wmo-consulent die inwoners helpt bij het vinden van passende ondersteuning. Je onderzoekt de hulpvraag, voert gesprekken en huisbezoeken en werkt samen met inwoners, zorgaanbieders en andere betrokken professionals aan duurzame oplossingen. De opdracht loopt van 19 oktober 2026 tot en met 1 januari 2027 , voor 36 uur per week . Er is een optie tot één keer 6 maanden verlenging . Wat ga je doen? Als Wmo-consulent ben je betrokken bij het volledige proces rondom Wmo-meldingen. Je werkzaamheden bestaan onder andere uit: - Beantwoorden van vragen en meldingen van inwoners en starten van het onderzoek. - Voeren van intakegesprekken en uitvoeren van onderzoek naar de ondersteuningsbehoefte. - Afleggen van huisbezoeken en beoordelen van de situatie bij de inwoner thuis. - Overleggen met zorgaanbieders en leveranciers van hulpmiddelen. - Bespreken met inwoners wat zij zelf kunnen doen en welke ondersteuning de gemeente kan bieden. - Organiseren en inzetten van passende hulp en ondersteuning. - Monitoren van afspraken en zorgtrajecten en waar nodig bijsturen. - Vastleggen van afspraken, plannen en onderzoeksresultaten. - Samenwerken met collega's binnen bijvoorbeeld schuldhulpverlening, jeugdhulp en de sociale basis. Je kijkt daarbij verder dan alleen de individuele hulpvraag. Je hebt oog voor de situatie van de inwoner, de mogelijkheden binnen het netwerk en de samenwerking met andere professionals. Een voorbeeld van je werkdag Je start de dag met een teamoverleg waarin binnengekomen meldingen worden besproken en verdeeld. Daarna stem je bijvoorbeeld af met collega's van schuldhulpverlening of de sociale basis over een gezamenlijke casus. Later overleg je met een casemanager Dementie over de ondersteuning van een echtpaar waarbij één partner dementie heeft. Samen maak je een plan met concrete afspraken en...
