Wmo-consulent Gemeente Functieomschrijving Wmo-consulent Wil jij inwoners helpen om zo zelfstandig mogelijk te leven en mee te doen in de samenleving? Voor een gemeente in Gelderland zoeken wij een ervaren Wmo-consulent voor 36 uur per week. Je behandelt uiteenlopende ondersteuningsvragen: van begeleiding en dagbesteding tot huishoudelijke hulp, woonvoorzieningen en vervoer. Tijdens huisbezoeken en keukentafelgesprekken onderzoek je samen met de inwoner welke ondersteuning nodig is. Je kijkt naar de persoonlijke situatie, de mogelijkheden binnen het eigen netwerk en passende voorzieningen. Je beheert je eigen caseload en werkt nauw samen met collega's en betrokken organisaties. Doordat je binnen een kleine gemeente werkt, is je werk veelzijdig en zijn de lijnen kort. Je kunt snel overleggen, denkt met elkaar mee en maakt gebruik van elkaars kennis. Je werkzaamheden bestaan uit: - Voeren van integrale keukentafelgesprekken en afleggen van huisbezoeken. - Onderzoeken en beoordelen van ondersteuningsvragen binnen de Wmo. - Stellen van indicaties voor begeleiding en dagbesteding. - Stellen van indicaties voor huishoudelijke hulp, woon- en vervoersvoorzieningen en collectief vervoer. - Schrijven van ondersteuningsplannen en gespreksverslagen. - Opstellen van zorgvuldig onderbouwde beschikkingen. - Afstemmen met begeleiders, betrokken instanties en collega's binnen het sociaal domein. Functieprofiel Wmo-consulent Je luistert aandachtig, stelt gerichte vragen en weet de ondersteuningsbehoefte van inwoners helder in kaart te brengen. Je schakelt gemakkelijk tussen verschillende soorten aanvragen en combineert betrokkenheid met een zorgvuldige beoordeling. Je werkt zelfstandig, neemt besluiten en zoekt de samenwerking op wanneer dat nodig is.
