Als Wmo consulent kijk jij verder dan de aanvraag. Ben jij een Wmo consulent met oog voor de mens achter de hulpvraag zonder de wettelijke kaders uit het oog te verliezen? Wij zoeken een Wmo consulent voor 24 uur per week. Wat ga je doen? Je houdt als consulent oog voor de bedoeling van de Wmo door inwoners te ondersteunen waar nodig zonder over te nemen wat zij zelf of met hun omgeving kunnen. Taken die daarbij horen zijn: - Je voert gesprekken met inwoners over hun hulpvraag en brengt de situatie integraal in kaart; - Je kijkt eerst naar wat iemand zelf kan, wat het sociale netwerk kan betekenen en welke algemene voorzieningen passend zijn; - Je beoordeelt of een maatwerkvoorziening vanuit de WMO noodzakelijk is en onderbouwt je besluiten zorgvuldig. Wie ben jij? Je bent betrokken, duidelijk, daadkrachtig en denkt in mogelijkheden. Je durft professioneel 'nee' te zeggen wanneer dat nodig is. Wat breng je mee? Werkervaring bij een gemeente is mooi meegenomen, maar geen vereiste. We vinden het veel belangrijker hoe je werkt. - Je hebt hbo-werk- en -denkniveau; - Je hebt kennis van de Wmo, relevante wet- en regelgeving; - Je hebt goede communicatieve vaardigheden; - Je werkt zelfstandig, maar kan ook goed samenwerken in een team.
