Je indiceert en organiseert de hulpverlening en/of interventies die passend zijn bij de situatie van de hulpvrager. Naast het behandelen/beoordelen van hulpvragen fungeer je als casemanager. Je houdt de vinger aan de pols met betrekking tot de ingezette hulpverlening, je evalueert het effect van de inzette interventies, je stelt acties waar nodig bij en je rapporteert over de voortgang. We zijn op zoek naar een ervaren Consulent Wmo GG Eisen Een afgeronde hbo bachelor opleiding in de richting van Maatschappelijk werk; Minimaal 3 jaar aantoonbare werkervaring in de afgelopen 5 jaar als Consulent WMO GG bij een gemeente Aantoonbare werkervaring met alle voorkomende GGZ aanvragen Aantoonbare kennis van Suite4sociaaldomein Competenties Snel en flexibel kunnen schakelen; Je kunt zelfstandig werken en zelf prioriteren; Je hebt passie voor het Maatschappelijk Domein; Je bent communicatief vaardig en klantvriendelijk; Je kunt rapporteren en gespreksverslagen schrijven en bent administratief goed onderlegd.
