Binnen de regio Flevoland wordt momenteel hard gewerkt aan de inrichting van een vernieuwd, regionaal toegangsproces. Jij speelt als Wmo Consulent Beschermd Wonen een belangrijke rol in dit hybride toegangsteam voor alle vijf de gemeenten. Vanuit de belofte van BMC Uitvoering sta je voor het leveren van duurzaam resultaat en het volledig ontzorgen van de opdrachtgever. Jouw inzet helpt direct om wachttijden en knelpunten in de zorg aan te pakken. In jouw rol ben je verantwoordelijk voor: - Het onderzoeken van meldingen en het bepalen van de best passende oplossing, zoals individuele begeleiding, dagbesteding of beschermd wonen. - Het zelfstandig voeren van je eigen caseload, van de eerste melding tot aan de beschikking. - Het beoordelen van aanmeldingen op volledigheid en het verhelderen van de ondersteuningsvraag door middel van gesprekken met de cliënt en diens verwijzer. - Het accuraat verwerken van onderzoeksgegevens en het afstemmen met specialisten indien het dossier daarom vraagt. - Het adviseren van het Multidisciplinair Overleg en het helder motiveren van je besluit (positief of negatief). - Het proactief signaleren van knelpunten in werkprocessen en dit aankaarten binnen het team. Functie-eisen - Je hebt een afgeronde hbo-opleiding in een sociaal-maatschappelijke of zorgrichting (bijvoorbeeld hbo-v, hbo-mw of SPH). - Je brengt minimaal 5 jaar relevante werkervaring mee met de doelgroep. - Je hebt actuele, inhoudelijke kennis van de Wmo en aanverwante wet- en regelgeving, en bezit paramedische kennis om dossiers medisch in te kunnen schatten. - Je hebt affiniteit met administratieve processen en werkt uiterst zorgvuldig met digitale reg...
