In deze rol ben je de schakel tussen inwoners en hulp op het gebied van wonen, welzijn en zorg. Je pakt zelfstandig aanvragen, onderzoeken en heronderzoeken op en zorgt voor maatwerk in iedere casus. Je werkt in een afwisselende functie waarin je professioneel advies geeft en samen met inwoners zoekt naar oplossingen die bijdragen aan zelfredzaamheid en participatie. Wat ga je doen - Je geeft inwoners professionele informatie, advies en ondersteuning op het gebied van wonen, welzijn en zorg. - Je behandelt aanvragen, onderzoeken en heronderzoeken binnen de Wmo. - Je stelt indicaties op voor voorzieningen zoals woningaanpassingen, dagbesteding, begeleiding, huishoudelijke hulp, rolstoelen en vervoer. - Je gaat in gesprek met inwoners, onderzoekt samen de hulpvraag en ondersteunt bij het realiseren van passende oplossingen. - Je werkt samen met externe partijen, zoals zorgaanbieders en leveranciers. - Je voert zelfstandig regie op je casussen en werkt toe naar heldere en goed onderbouwde besluiten. Over jou - Je werkt zelfstandig en bent in staat om na een korte inwerkperiode direct verantwoordelijkheid te nemen. - Je bent besluitvaardig en neemt ook onder druk snel en goed onderbouwde besluiten. - Je hebt een sterk analytisch vermogen en werkt aanvragen uit tot heldere besluiten. - Je beschikt over een motiverende en coachende gespreksstijl. - Je luistert goed, stelt de juiste vragen en stimuleert inwoners om eigen verantwoordelijkheid te nemen en hun netwerk te benutten. - Je denkt creatief en kijkt verder dan standaardprocedures om passende oplossingen te vinden. Eisen - Je beschikt over hbo werk- en denkniveau. - Je voldoet aan de in de functiebeschrijving genoemde opleidingseis. - Je bent in staat om half oktober te starten met de opdracht. - Je bent in staat zelfstandig aanvragen op te pakken. - Je bent beschikbaar voor 36.
