De woon - trajectbegeleider ondersteunt en begeleidt gezinnen, kinderen en jongeren op een integrale manier binnen een krachtgericht kader waarin het versterken van vaardigheden centraal staat. De begeleiding gebeurt via een groepswerking in een leergroep + individuele ondersteuning + ondersteuning van de context.

Als integrale begeleid(st)er ga je samen met het gezin, kind en netwerk op pad om vanuit hun eigen krachten ondersteuning te bieden binnen een opvoedend kader. Deze integrale aanpak focust zich op vier grote pijlers:

o Groepsbegeleiding

o Individuele begeleiding van het kind/de jongere

o Contextbegeleiding

o Schoolbegeleiding

· Je hecht belang aan continue interactie , inspraak en afstemming tussen context , het team en de leergroep.

· Je neemt deel aan interne en externe overlegmomenten.

· Je verzorgt administratie en verslaggeving.

· Je werkt hiervoor samen met je collega’s.

· Je werkt vanuit de LIA kernwaarden : engagement, respect, transparantie, eigenheid en autonomie, samenwerking en open communicatie.

· Je hebt een bachelor opleiding in een menswetenschappelijke richting.

· Bonus als je kennis hebt van diverse theoretische kaders zoals traumasensitief werken, leertheoretische principes en gedragstherapeutische technieken, de systeemtheorie en het oplossings -en herstelgericht werken.

· Je bent empathisch, geduldig en oprecht.

· Je bent creatief in denken en doen en neemt initiatief.

· Je bezit gevoel voor humor en een positieve ingesteldheid.

· Je bent stressbestendig.

· Je bent een teamspeler en kan open en transparant communiceren.

· Je bent bereid om een paar avonden per week te werken tot 19u.

· Je beschikt over een rijbewijs B.
