32-36 uur uur Heb jij als zorgprofessional hart voor cliënten met verschillende ziektebeelden? Als woonbegeleider bij BW Sliedrecht ben jij de persoonlijk begeleider van cliënten met diverse psychiatrische problematieken. Je bent intensief betrokken bij de dagelijkse ondersteuning van cliënten, waarbij herstelgericht werken centraal staat. In verbinding met het team maak jij het verschil voor cliënten. Aandacht voor persoonlijke groei Met aandacht voor wat er nodig is creëer jij voor al jouw cliënten een prettige woon- en leefsituatie. Je werkt herstelgericht en ondersteunt de cliënt bij (medische) afspraken en bij het versterken van het eigen netwerk. Jouw doel is steeds om elke cliënt een zinvol bestaan te bieden door te helpen waar nodig en te motiveren om het zelf te doen waar dat kan. Samen met het team ontwikkel je de zorg en jezelf om de cliënt nog beter te kunnen helpen. Wat je verder gaat doen: - Je komt terecht in een hecht en divers team. - Je stelt zorgplannen op, samen met de cliënt en het netwerk. - Je geeft medicatie volgens het zorgplan. - Je voert nauw overleg met FACT, Indigo of huisarts over het zorgplan. - Je ziet toe op het psychisch en fysiek welzijn van de cliënten. - Je werkt herstelgericht en zoekt per cliënt uit wat er mogelijk is. - Je stimuleert cliënten in ontwikkeling, beweging en ontspanning. - Je betrekt cliënten waar nodig bij op de woon- en leefsituatie gerichte activiteiten. - Je begeleidt en ondersteunt cliënten bij het uitvoeren van diverse (dagelijkse) activiteiten. - Je hebt een aantal extra overkoepelende taken binnen het team. - Je volgt een aantal verplichte cursussen, zoals BHV. Jouw werkplek Je gaat impact maken binnen BW Sliedrecht, een beschermd wonen locatie met 24/7-begeleiding voor 33 cliënten.
