Dit ga je doen BW Pijnackerstraat is een beschermde woonvorm in Rotterdam in de wijk Oude Noorden, waar 18 cliënten wonen. De cliënten bij BW Pijnackerstraat hebben te maken met psychiatrische problematiek, waar alcohol en verslaving op de voorgrond kan staat. De begeleiding is gericht op stabilisatie en kijken welke mogelijkheden er zijn om door te groeien naar zelfredzaamheid. De werkzaamheden: - observeren van de cliënten, signaleren van veranderend gedrag waardoor je tijdig verandering kan herkennen en daarin treed je accuraat op; - je begeleidt cliënten in hun hersteltraject, samen met de cliënt schrijf je diens herstelplan, dit doe je via de SMART methoden en 1x in de 6 maanden evalueer je samen met de cliënt en diens naasten de doelen die gesteld zijn; - je werkt volgens de IRB methodiek en registreer en rapporteer jouw bevindingen in het daarvoor bestemde EPD; - minimaal 1x in de week voer jij een persoonlijk begeleider gesprek met de cliënten waar jij persoonlijk begeleider van bent. Dit ben jij - je werkt graag in 16- uurs diensten; - ervaring met de doelgroep; - je kunt grenzen stellen; - je handelt vanuit rust; - je hebt een afgeronde mbo-3 of mbo-4 opleiding in de richting Maatschappelijke Zorg, Sociaal Werk of een vergelijkbare opleiding; Om in aanmerking te komen voor deze functie is een positieve Verklaring Omtrent Gedrag (VOG) vereist.
