The posting

In onze woning “Villa Bevrijdingslaan” in Avelgem wonen volwassenen met een diverse ondersteuningsvraag. In een 24/7 groepsondersteuning bieden we nabijheid, medische zorg en permanentie aan 8 volwassenen. De meeste cliënten hebben een niet- aangeboren hersenletsel. Daarnaast ondersteun je ook 6 cliënten die in een studio in de Bevrijdingslaan wonen. De belangrijkste opdracht als wooncoach hierin is organiseren, coördineren, faciliteren en verbinden. Bij ons staat de mens centraal: we luisteren naar de wensen, behoeften en mogelijkheden van elke cliënt. Samen met familie en andere betrokkenen bouwen we aan een persoonlijke en respectvolle zorg.

Als wooncoach ben je een belangrijke schakel in ons team. Je ondersteunt cliënten in hun dagelijks leven en speelt een actieve rol in het observeren en opvolgen van hun noden. Als mentor volg je enkele individuele cliënten van heel nabij op en onderhoud je contacten met hun familiaire en professionele netwerk.

  • Flexibiliteit is voor jou vanzelfsprekend: je past je begeleidingsstijl aan naargelang de doelgroep. Ochtend-, avond- en weekendwerk passen binnen jouw ritme. De nachtshiften worden opgevangen door het team van nachtcoaches.
  • Ervaring in verpleegkunde is een meerwaarde.
  • Je beschikt over een rijbewijs B.
