Bovendien ga je graag aan de slag in een team waar jouw expertise écht het verschil maakt. De poli ouderen in Gouda zoekt een zelfstandige en ondernemende specialist ouderengeneeskunde. Als specialist ouderengeneeskunde speel je een belangrijke rol in het behandelen en begeleiden van patiënten met complexe problematiek en daarmee het verbeteren van de levenskwaliteit. Als regiebehandelaar geef je vorm aan de zorg op het gebied van diagnostiek en behandeling. Ook zie je toe op de behandeling van somatische, psychische en cognitieve ziektebeelden. Jij weet de juiste balans te vinden tussen medische zorg, welzijn en kwaliteit van leven. Dit doe je in overleg met de patiënt en familie. En verder? - Stel je het behandelplan op, coördineer je de uitvoering ervan en voer je de regie over met het multidisciplinaire team en ben je eindverantwoordelijke voor de medische behandeling inclusief het voorschrijven van medicatie; - Onderhoud je contacten met verwijzers, externe behandelaren, functionarissen in GGZ instellingen of andere hulpverleners over individuele cliënten; - Geef je werkbegeleiding en/of supervisie en lever je een bijdrage aan opleidingsactiviteiten en deskundigheidsbevordering; - Heb je een consultatieve functie ten behoeve van in- en externe collega's specialist ouderengeneeskunde en ook aan psychiaters binnen de organisatie. Wie ben jij? Om de functie van specialist ouderengeneeskunde goed uit te voeren, beschik je over: - Een afgeronde opleiding tot specialist ouderengeneeskunde; - Affiniteit, kennis en ervaring met de doelgroep; - BIG-registratie; - AGB-code of kun je deze aanvragen.
