Werk dat ertoe doet Salland Zorgkantoor is verantwoordelijk voor de uitvoering van de Wet langdurige zorg (Wlz) in onze regio. Samen met cliënten, hun naasten, zorgaanbieders en andere partners werken we eraan dat mensen die langdurige en vaak complexe zorg nodig hebben passende zorg krijgen. Met een team van ongeveer 25 collega's zetten we ons hier iedere dag voor in. Deskundigheid, betrokkenheid en samenwerking staan daarbij centraal. Jouw rol als Zorgadviseur Als Zorgadviseur heb je een veelzijdige functie op het snijvlak van zorg, wet- en regelgeving en persoonlijk contact. Je helpt cliënten en budgethouders bij het organiseren van passende langdurige zorg. Daarbij ben je een belangrijk aanspreekpunt voor cliënten, vertegenwoordigers, zorgaanbieders en andere partijen in de regionale zorgketen. Een belangrijk onderdeel van je werk is het beoordelen van aanvragen voor een persoonsgebonden budget (pgb). Je voert bewust-keuzegesprekken, beoordeelt zorgovereenkomsten en gaat bij budgethouders op huisbezoek. Je kijkt daarbij verder dan de administratieve kant. Je beoordeelt of de zorg passend en doelmatig wordt ingezet, of de budgethouder voldoende regie kan voeren en of het pgb volgens de geldende wet- en regelgeving wordt gebruikt. Daarnaast houd je je bezig met complexe zorgsituaties. Je analyseert wat er speelt, weegt verschillende belangen zorgvuldig af en zoekt samen met cliënten en andere betrokkenen naar pas...
