Samen zetten we ons elkedag in voor toegankelijke en betaalbare zorg voor iedereen.Bij CZ Zorgkantoor help je mensen de weg te vinden binnen de Wet langdurige zorg (Wlz). Alszorgbemiddelaar ben jij het eerste aanspreekpunt voor cliënten, hun naasten en zorgaanbieders.Als zorgbemiddelaar bestaat een groot deel van je werk uit telefonisch contact. Je bent dagelijks ingesprek met cliënten, hun naasten, cliëntondersteuners en zorgaanbieders. Je beantwoordt vragen overde Wlz, brengt zorgsituaties in kaart en begeleidt cliënten bij het vinden van passende langdurige zorg.Daarbij denk je actief mee over mogelijkheden en zoek je samen met betrokkenen naar passendeoplossingen. Wanneer passende zorg niet direct beschikbaar is, neem jij de regie in de bemiddeling. Je stemt afmet zorgaanbieders, onderzoekt alternatieven en brengt zorgaanbieders bij elkaar om ervoor tezorgen dat cliënten de juiste zorg op de juiste plek ontvangen. Geen dag is hetzelfde. De ene keer help je een mantelzorger die dringend ondersteuning nodigheeft, de andere keer overleg je met een zorgaanbieder over beschikbare plaatsen of begeleid jeeen complexe bemiddelingsvraag. Hierbij werk je nauw samen met collega's en andereketenpartners. Naast de telefonische contacten leg je afspraken, zorgvragen en acties vast in onze systemen. Jeverwerkt berichten van cliënten en zorgaanbieders, beoordeelt aanvragen en zorgt ervoor datdossiers volledig en actueel zijn. Ook houd je de voortgang van bemiddelingen in de gaten, stem je af met collega's enzorgaanbieders en zorg je ervoor dat gemaakte afspraken goed worden vastgelegd en opgevolgd.Daarbij maken we steeds vaker gebruik van digitale hulpmiddelen en AI....
