Als zorgcoördinator heb je onder meer de volgende taken:

  • De burger van Hulshout ondersteunen en begeleiden bij de zoektocht in het zorglandschap.
  • Samen met team welzijn een lokaal sociaal beleid en welzijnsbeleid uitwerken
  • Projecten ontwikkelen en coördineren voor thema's zoals gezondheid en preventie, buurtgerichte zorg en iedereen digitaal.
  • Opzetten en opvolgen van preventieve activiteiten om de cliënten de kansen en mogelijkheden te bieden om zo lang mogelijk in eigen omgeving te kunnen blijven wonen.

Een goede zorgcoördinator beschikt over de volgende zaken:

  • Bachelordiploma (of daarmee gelijkgesteld; kandidaten zonder bachelordiploma leggen een capaciteitstest af)
  • Projectmatige vaardigheden
  • Goede luistervaardigheden en in staat om effectief te communiceren
  • Een passie voor het helpen van anderen in moeilijke situaties
