Zorgcoördinator ARTS / Geneeskundig Floormanager GGD Flevoland - Almere - 31-12-9999 In Almere werken we met een zorgcoördinatievoorziening, waar we willen bijdragen aan een toekomstbestendige in-, door- en uitstroom van de acute zorgketen in Flevoland en Gooi en Vechtstreek. Op deze voorziening worden er lessen geleerd; hoe gaan de triageprofessionals met elkaar samenwerken? Welke triageprocessen- en systemen zijn passend bij sector overstijgende zorgcoördinatie? Gaat de zorgcoördinatievoorziening ook zorgprofessionals in de frontlinie helpen bij coördinatie van zorg? Hebben wij andere huisartsenposten en zorgsectoren nodig om de acute zorgketen sluitend te maken? Om deze doorontwikkeling gestalte te geven zoeken wij een arts op de zorgcoördinatievoorziening! ZORGCOÖRDINATIE ARTS/ GENEESKUNDIG FLOORMANAGER Inzet in overleg/standplaats Almere Maak impact in de acute zorgketen Ben jij arts en wil je niet alleen deelnemen aan deze ontwikkeling en verandering, maar deze ook helpen vormgeven? Dan is dit jouw kans. Als zorgcoördinatie-arts/ geneeskundig floormanager vervul je een nieuwe, sector overstijgende rol binnen de acute zorgketen. Je brengt professionals, disciplines en organisaties samen, helpt nieuwe werkwijzen ontwikkelen en draagt bij aan betere samenwerking tussen huisartsenspoedpost, meldkamer ambulancezorg en ketenpartners. Je krijgt de unieke kans om een functie én werkwijze mede vorm te geven en daarmee blijvende impact te maken op de acute zorg in de regio Flevoland en Gooi en Vechtstreek.
