Je kijkt niet alleen naar wat er vandaag nodig is, maar ook naar de volgende stap in iemands herstel. Je houdt overzicht, brengt mensen bij elkaar en zorgt dat plannen ook echt uitgevoerd worden. Wat ga je doen? Je bent verantwoordelijk voor het coördineren van zorgtrajecten van cliënten die ondersteuning krijgen binnen beschermd wonen. Je voert intakes, stelt samen met cliënten begeleidingsplannen op en bewaakt de voortgang. Daarbij kijk je verder dan één hulpvraag. Je onderzoekt wat iemand nodig heeft om weer verder te kunnen en schakelt waar nodig met collega's en andere betrokken organisaties. Je bent geen manager, maar neemt wel regie. Je ondersteunt begeleiders bij inhoudelijke vragen, denkt mee in lastige casussen en helpt collega's om samen tot de beste aanpak te komen. Onze doelgroep is divers. Veel cliënten hebben psychiatrische problematiek en regelmatig is er ook sprake van autisme. Daarom is het fijn als je daar ervaring mee hebt of nieuwsgierig bent om je daarin verder te ontwikkelen. Niet omdat je overal een antwoord op hoeft te hebben, maar omdat je begrijpt hoe belangrijk maatwerk, duidelijkheid en een goede samenwerking zijn. Waarom deze functie?
