Ben jij de zorgcoördinator die we zoeken? Samen maken we het verschil Bij de Nederlandse Obesitas Kliniek werken professionals die het verschil maken. Als patiënten bij ons komen, hebben ze zelf alles geprobeerd om af te vallen. Wij zijn er om hun leven een positieve wending te geven. Dit doen we door onze patiënten intensief te begeleiden bij de benodigde leefstijlverandering. Aan hen de taak om, met onze ondersteuning, het nieuwe gedrag vast te houden. Als zorgcoördinator bij de NOK maak je samen met de multidisciplinaire teams het verschil tussen een ongezonde en een gezonde leefstijl van onze patiënten. Op iedere NOK-vestiging werken een aantal van deze teams. Ze bestaan uit een internist, chirurg, psycholoog, arts, diëtist en bewegingsdeskundige. Elk team werkt onder leiding van een vestigingscoördinator en wordt ondersteund door de zorgcoördinator. NOK team Suzanne over haar werk als zorgcoördinator Suzanne Als zorgcoördinator bij de NOK: - verzorg je de oriënterende groepsvoorlichting (algemene informatie tijdens eerste kennismaking) met patiënten. - begeleid je patiënten tijdens de screening. - kun je in voorkomende gevallen patiënten wegen en meten. - verzorg je algemene schriftelijke correspondentie (incl. e-mail) rondom het traject en boek je patiënten in in het patiëntvolgsysteem. - ben je telefonisch en fysiek het eerste aanspreekpunt voor patiënten, verwijzers, zorgverzekeraars en samenwerkingspartners. - ondersteun je de vestigingscoördinator bij de patiëntenplanning. - heb je contact met verwijzers, zorgverzekeraars en samenwerkingspartners. - heb je overleg met teamleden, collega-zorgcoördinatoren en de vestigingscoördinator. - neem je deel en draag je bij aan periodieke locatieoverleggen. - neem je deel en draag je bij aan monodisciplinaire overleggen (periodiek organisatiebreed overleg van alle zorgcoördinatoren).
