- Woonplaats: buitengebied Ede Barneveld - E-mail: Stuur een bericht Voor een client (51) in het buitengebied tussen ede en barneveld zoek ik ter uitbreiding van haar team een (post-) HBO geschoolde zorgprofessional die de regiefunctie op zich wil nemen. Dit houdt in vinger aan de pols bij de maandelijkse evaluatie en 2 x per jaar een evaluatiegesprek waarbij het plan wordt geactualiseerd en de bereikte doelen worden besproken. Ook kan de zorgprofessional bij uitvallen van de zorgmedewerker (MBO4) als back-up worden gevraagd. Het energielevel van cliënt is laag waarbij pijn en vermoeidheid, problemen met fijne motoriek en soms uiteindelijk uitval afhankelijk van vermoeidheid speelt. Cliënt verdraagt qua prikkels bij overbelasting steeds minder bv drukte aanraking, geluid-licht lucht. Diens sociale activiteiten zijn daardoor beperkt, maar cliënt geniet van haar dieren, de tuin enz De wekelijkse begeleiding van de zorgmedewerker bestaat uit hulp bij digitale administratie, bij gebrek aan digitale handigheid. Om de beeldschermwerk en prikkels te verminderen wordt zij graag geholpen om dingen te voor selecteren iets te regelen en informatie te vinden. Er ligt veel opruimwerk daarnaast is bespreken van zorgdoelen ook belangrijk. Wat kan jij: * De (post-) HBO geschoolde zorgprofessional is aantoonbaar betrokken bij continuatie en realiseren van de gestelde doelen in het reeds bestaande ondersteuningsplan. * De zorgprofessional houdt zicht op de in het Plan van Aanpak opgenomen SMART geformuleerde resultaten/doelen of en hoe de doelstellingen worden bereikt.. * Kan volgens de PGB-eisen van de Gemeente Ede werken * De zorgprofessional is in staat om de ondersteuning af te stemmen en samen te werken met het netwerk, voorliggende voorzieningen en eventuele andere zorgaanbieders. * De zorgprofessional maakt adequate afspraken over de samenwerking en communicatie. * Een relevante voltooide (post-) HBO opleiding * Digitaal vaardig met ...
