Zorgregisseur Verpleging Als zorgregisseur verleen je niet alleen verpleegkundige zorg, maar houd je vooral rekening met de behoeftes en gewoonten van de bewoners. Deze mensgerichte zorg draait om de levenskwaliteit van de bewoners en hun naasten. Jouw zorg staat altijd in het teken van de bewoner. Wat ga je doen? - Je levert een belangrijke bijdrage aan de zelfstandigheid van onze bewoners. - Je verleent verpleegkundige zorg en verzorging. - Je combineert in je functie zorgtaken met coördinerende werkzaamheden. - Je biedt klantgerichte verpleging, verzorging, begeleiding en voorlichting met het zorgplan als uitgangspunt. - Je bewaakt de voortgang en de kwaliteit van zorg. - Je coördineert de zorg in overleg met de cliënt en onderhoudt contacten met familie en andere zorgverleners. - Je bent een belangrijk aanspreekpunt voor je team en - Je werkt vanzelfsprekend goed samen met bewoners, hun naasten en je collega's en vrijwilligers. Waar ga je werken? Waerthove in het centrum van Sliedrecht is een van onze grote locaties waar kleinschalige, individuele zorg geboden wordt. Je gaat werken op afdeling de Giessen, hier wonen 40 bewoners waarvan 32 bewoners met Gerontopsychiatrie en 8 bewoners somatiek. 32 bewoners vanaf 40 jaar die aangewezen zijn op chronisch psychiatrische verpleeghuiszorg (CPV). Rivas locatie Waerthove is aangesloten bij de vereniging van Gerontopsychiatrie en sluit aan bij het landelijk zorgprogramma. Wie ben jij? - Je hebt een diploma verpleegkundige op mbo4-niveau of enkele jaren ervaring als verzorgende IG. - Je hebt de opleiding zorgregisseur afgerond of bent bereid deze te volgen. - Je bent BIG-geregistreerd (verpleegkundige) en Hepatitis B gevaccineerd. Bij ons kan je rekenen op: - Je ontvangt een salaris in schaal 45 (cao VVT) trede zij-instroomperiodiek € 3157,37 per maand op basis van 36 uur.
