Zorgregisseur NAH Functie omschrijving Ben jij de zorgregisseur die zelfstandigheid en eigen regie bij bewoners stimuleert, en weet jij je team op een juiste manier te coachen? Dan zijn wij op zoek naar jou! Wat je doet Als zorgregisseur bij InteraktContour ben jij de spil in de zorg voor mensen met niet-aangeboren hersenletsel (NAH). Jij coördineert het zorgproces, bewaakt de kwaliteit en zorgt ervoor dat bewoners de ondersteuning krijgen die ze nodig hebben om zo zelfstandig mogelijk hun eigen leven te leiden. Samen met een andere zorgregisseur-collega ben je het aanspreekpunt op locatie voor bewoners, collega's en samenwerkingspartners en hebt een actieve rol in het betrekken en coachen van je team bij de uitvoering van de zorgplannen. Daarbij heb je de ruimte om zelf richting te geven aan je rol en de samenwerking in het team vorm te geven, terwijl de uiteenlopende zorgvragen zorgen voor veel afwisseling in je werk. Je doet veel meer dan alleen zorgplannen opstellen en bijhouden. Je denkt mee over het vormgeven en organiseren van teamoverleggen, bewonersbijeenkomsten en andere initiatieven die bijdragen aan een stabiele en fijne woonomgeving. In nauwe samenwerking met verpleegkundigen en gedragswetenschappers bepaal je wie welke taken oppakt, zonder dat jij zelf medische handelingen uitvoert. Je schakelt snel bij veranderende zorgbehoeften en zorgt ervoor dat alle afspraken zorgvuldig worden vastgelegd in het zorgdossier. Wil jij impact maken in een veelzijdige en coördinerende functie?
