The posting

Als trajectbegeleider behoud je het overzicht over het traject van de cliënt en de hulpverlening die hierrond wordt georganiseerd. Je hebt hierin een inhoudelijk sturend mandaat.

Je geeft samen met de individuele begeleider vorm aan het inhoudelijke traject van de cliënt en gaat hierbij actief op zoek naar (multidisciplinaire) samenwerking, zowel binnen als buiten de voorziening.

Je coördineert het IHP en bent verantwoordelijk voor de inhoud, verslaggeving en communicatie hierover.

Je bewaakt de contacten met de ruimere context van de cliënt en bent verantwoordelijk voor de opmaak en opvolging van het cliëntendossier, van intake tot uitstroom.

Deze functie omvat taken op verschillende niveau's:

Beleidsniveau

  • Samen met de collega’s trajectbegeleider en hoofdbegeleider en clusterverantwoordelijke maak je deel uit van het begeleidingsteam.
  • Op dit niveau neem je samen met de collega's clustergebonden beslissingen.
  • De trajectbegeleider heeft geen leidinggevende functie en ontvangt leiding van de clusterverantwoordelijke.

Leefgroepsniveau:

  • Je ondersteunt het cliëntsysteem en verfijnt samen met de begeleiders de werking binnen de leefgroepen. Dit vanuit een sociaal-emotioneel en contextueel denkkader.
  • Je bewaakt de inhoudelijke implementatie van IHP-afspraken in de leefgroep (in tandem met IB en HB).
  • Het is je taak het orthopedagogisch klimaat binnen het team inhoudelijk te ondersteunen en samen met de teamleden concreet gestalte te geven.
  • Je volgt het cliëntgebeuren mee op via teamvergaderingen, huisbezoeken, overlegmomenten, …
  • Je stemt het handelen af op de ondersteuningsvragen van de cliënt en zijn/haar omgeving Respect hebben voor de cliënt en zijn/haar ruimere context en de ervaringsdeskundigheid van ouders
  • Je stimuleert een positieve feedbackcultuur en laat de collega's nadenken over hun aanpak en handelen.

Organisatieniveau

  • Je geeft mee vorm aan het opnamebeleid van de voorziening.
  • Je beslist in overleg met het begeleidingsteam welke jongere instroomt of doorstroomt naar de leefgroepen binnen de cluster.
  • Je engageert je om vanuit onze waarden en christelijke inspiratie op een positieve en respectvolle manier samen te werken met bewoners, hun netwerk en collega's
  • Je hebt bij voorkeur ervaring met de doelgroep: jongeren met een licht tot matig verstandelijke beperking en bijkomende problematieken, zoals gedrags- en emotionele stoornissen (GES), autismespectrumstoornissen (ASS) en trauma.
  • Ervaring in het samenwerken met ouders, familieleden, wettelijke vertegenwoordigers en andere betrokkenen strekt tot aanbeveling.
  • Je beschikt over een positieve en constructieve houding, sterke communicatieve en administratieve vaardigheden en een goed organisatietalent.
  • Je bent in staat kritisch te reflecteren over je eigen handelen en staat open voor feedback.
  • Je hebt gevoel voor humor en weet dit op een gepaste manier in te zetten.
  • Je beschikt over een relevant bachelor- of masterdiploma in een sociaal-agogische of menswetenschappelijke richting
  • Je beschikt over een blanco-attest model 2.
