Bienvenue au sein de la Fondation Œuvre de la Croix Saint-Simon.

Acteur majeur dans les secteurs sanitaires et médico-sociaux aux services des personnes vulnérables de la naissance à la fin de vie.

Notre projet institutionnel est de proposer une approche globale et diversifiée d’accueil, d’accompagnement, de soins ou de prise en charge médicale en Ile-de-France.

Situé dans le 11ème arrondissement de Paris, l’Hôpital Mère Enfant de l’Est Parisien est un service de soins de suite qui accueille et accompagne des femmes enceintes et des mères avec leur bébé de moins d’un an qui présentent des difficultés dans l’instauration des liens précoces mère-enfant.

L’hospitalisation à temps plein permet aux femmes de tisser un lien mère-nourrisson dans un contexte propice et sécurisant.

Notre service est un lieu de soins qui conjugue les soins somatiques, psychologiques, psychiatriques, les soins des interactions mère-enfant, père-enfant et l’accompagnement socio-éducatif des familles.

Vos responsabilités

Rattaché(e) à la directrice des soins, au sein d’une équipe pluridisciplinaires, vous avez pour principale mission d’élaborer et de soutenir un projet individuel éducatif et parental dans le respect du projet de soins et élaboré en concertation avec la patiente et l’équipe de soins.

Dans le cadre de votre mission, vous être en charge de :

· Mettre en place le projet éducatif personnalisé et ajustement tout au long de l’hospitalisation

· Veiller au bien-être de la patiente et de son bébé

· Participer à l’évaluation socioéducative en binôme avec l’assistante sociale

· Aider la personne à formuler ses attentes, ses objectifs et ses besoins

· Soutenir la relation parent-enfant

· Assurer une continuité du lien entre le patient et l’institution

· Mettre en œuvre d’ateliers thérapeutiques centrés sur les problématiques de la vie sociale et relationnelle

· Accompagner la mère à se sociabiliser par des accompagnements extérieurs individuels et collectifs

· Accompagner la mère dans certaines démarches administratives

· Participation à la préparation à la sortie d’hospitalisation

· Assurer la traçabilité des actions menées

· Participer aux réunions institutionnelles, inter-institutionnelles et aux groupes de travail

Vous avez

- Un diplôme d’Etat d’Educateur Spécialisé.

- Le sens des responsabilités, de l’organisation.

- Un esprit curieux et une personnalité dynamique.

- Des capacités d’adaptation à l’environnement, aux habitudes de vie du patient et son entourage.

- Des compétences reconnues à l’évaluation des urgences.
