Vos missions : Coordonner, Expertiser, Accompagner

Au sein de notre pôle Mère-Enfant, vous êtes le garant de la sécurité et de la continuité des soins sur le secteur de pédiatrie

· Coordination Médicale : Vous validez les admissions (codification T2A) et les sorties en lien avec le médecin référent (validation des synthèses de prise en charge). Garant du projet thérapeutique, vous en assurez la validation, l’actualisation et la mise en œuvre en étroite collaboration avec les cadres de coordination, les médecins hospitaliers et les praticiens de ville. En partenariat avec les médecins référents, vous assurez les prescriptions nécessaires et le suivi biologique en temps réel, dont vous garantissez la transmission aux correspondants et à l’équipe soignante.

· Appui Médical et Expertise de Terrain : Interlocuteur privilégié des équipes soignantes, vous réalisez des visites à domicile dès que nécessaire

· Stratégie et Développement : Force de proposition, vous explorez de nouveaux champs de prise en charge et définissez les moyens nécessaires à la mise en œuvre des projets. Vous portez le projet médical au sein du projet d’établissement de l’HAD.

· Partenariats Territoriaux : Vous tissez et entretenez des liens solides avec les partenaires du territoire (Hôpitaux, CAMSP, SESSAD, PMI, RIFHOP et libéraux) pour promouvoir notre modèle de soins.

Votre profil

· Docteur en médecine impérativement inscrit au Conseil de l’Ordre des médecins spécialisé en Pédiatrie.

  • Votre expertise : Vous maîtrisez les spécificités de l’exercice hospitalier et libéral.
  • Votre tempérament : Curieux, dynamique, et doté d'un excellent relationnel, vous appréciez le travail collaboratif et la complexité des soins à domicile.
