Work Arrangement

  • Home visit / community-based role
  • 5-day or 5.5-day work week depending on operational requirements

Job Overview

We are looking for a compassionate and dedicated Staff Nurse to join our Home Care Services team. In this role, you will provide nursing care to clients in their homes, support care planning and coordination, and work closely with caregivers and other healthcare professionals to ensure quality care delivery.

Key Responsibilities

Clinical Care

  • Provide nursing care to clients in home and community settings
  • Perform nursing procedures such as wound dressing, medication management, catheter care, stoma care, and tube feeding as required
  • Monitor and assess clients' health conditions and report any changes promptly
  • Administer medications and treatments in accordance with nursing standards and care plans
  • Educate clients and caregivers on care needs and home management

Care Planning & Coordination

  • Conduct nursing assessments and support the development of care plans
  • Coordinate care with doctors, therapists, caregivers, and other healthcare professional
  • Support discharge-to-home and long-term care arrangements where require
  • Participate in case discussions and care reviews

Documentation & Compliance

  • Maintain accurate and timely clinical documentation
  • Ensure nursing care is delivered according to clinical guidelines and company policies
  • Adhere to infection control, patient safety, and regulatory requirements
  • Report incidents, concerns, and clinical risks appropriately

Client & Caregiver Support

  • Provide guidance and support to clients and family members
  • Promote health education and self-care where appropriate
  • Support caregivers in carrying out care plans safely and effectively

Team Support

  • Work closely with Healthcare Assistants, Caregivers, and Operations teams to support service delivery
  • Participate in team meetings, training, and continuous learning activities
  • Provide basic guidance to junior staff and caregivers when required

Requirements

  • Registered Nurse with a valid Singapore Nursing Board (SNB) practising certificate
  • Minimum 2 to 3 years of nursing experience
  • Experience in community care, home care, hospital, outpatient, or ILTC settings will be an advantage
  • Good clinical assessment and nursing skills
  • Strong communication and interpersonal skills
  • Able to work independently and manage home visits effectively
  • Compassionate, patient, and service-oriented
  • Comfortable using digital systems for documentation
