Se busca trabajador/a social que desarrolle, coordine y evalúe el Servicio Ikaria de apoyo a la toma de decisiones y al ejercicio de la capacidad jurídica desarrollado por la Fundación Kyrios. Incluye el diseño, la coordinación y la supervisión de los sistemas de apoyo a la toma de decisiones de personas con funcionamiento intelectual límite, promoviendo su autonomía, su vida independiente, su autodeterminación y el respeto de sus derechos, en el marco del modelo de apoyos centrado en la persona. Esta misión se desarrolla en coherencia con los compromisos adquiridos por la Fundación como entidad designada para ejercer medidas de apoyo, ya sean de carácter privado, notarial o judicial, conforme a lo establecido en la Ley 8/2021, con la supervisión de la Dirección de Intervención Social. Requiere la gestión documental relacionada con los distintos ámbitos de la vida de la persona, la coordinación con recursos y entidades implicadas, y el ejercicio del apoyo conforme a la medida establecida. Las funciones específicas del puesto son, entre otras: -Diseñar, desarrollar, coordinar y evaluar, junto con la persona y su red de apoyo, un Sistema de Apoyo a la Toma de Decisiones (SATD), asegurando que refleje su voluntad, deseos y preferencias. -Informar, orientar y gestionar la formalización del SATD mediante los instrumentos jurídicos adecuados (documento interno, escritura pública notarial o resolución judicial). -Ejercer como profesional de referencia de las personas atendidas en el Servicio Ikaria. -Prestar apoyo a la toma de decisiones en los distintos ámbitos de la vida de la persona, incluyendo el cuidado de la salud física y mental, la participación social y comunitaria, la gestión económica y patrimonial, y otros aspectos relevantes, registrando las actuaciones en el sistema de gestión y activando las salvaguardas necesarias. -Coordinarse con la familia y con los miembros de la r.Para acceder al detalle de la oferta haga <a href="https://empleate.gob.es/empleo/#/oferta/1849005926" rel="nofollow">click aquí</a>
