På Jægersborghave Plejeboliger bliver du en del af en specialiseret demensafdeling, hvor personcentreret omsorg, høj faglighed og samarbejdsrelationer er fundamentet for vores arbejde.

  • Fast dagvagt, timeantal aftales
  • Weekendarbejde hver 2. eller hver 3. weekend – du vælger det, der passer bedst til dig
  • Tæt samarbejde med faglige koordinatorer, praksisnær demenskonsulent, udviklingssygeplejerske og plejehjemslæge
  • Arbejde med forankring af personcentreret omsorg og udvikling

Som social- og sundhedsassistent hos os

Som social- og sundhedsassistent er du en central fagperson i arbejdet med at omsætte personcentreret omsorg til praksis. Med dine socialpædagogiske kompetencer din sundhedsfaglige viden er du med til at sikre trivsel og skabe sammenhængende forløb med udgangspunkt i den enkelte beboers livshistorie, ønsker og behov.

Du bliver en del af et stærkt fagligt fællesskab, hvor du samarbejder tæt med faglige koordinatorer, kolleger på tværs af dag-, aften- og nattetimerne samt vores tværfaglige samarbejdspartnere.

Hos os får du blandt andet mulighed for at:

  • være kontaktperson for 3-4 beboere og koordinere en personcentreret indsats med udgangspunkt i beboerens ønsker, behov og livshistorie.
  • observere, vurdere og handle på ændringer i beboerens tilstand samt bidrage til høj sundhedsfaglig kvalitet.
  • deltage i beboerkonferencer og tværfaglige drøftelser sammen med faglige koordinatorer, demenskonsulent og plejehjemslæge.
  • arbejde ud fra Tom Kitwoods principper om personcentreret omsorg og bidrage til uddannelsen af kommende Social og sundhedsassistent elever og sygeplejestuderende.
