Je kunt rekenen op:

  • Direct een dienstverband voor onbepaalde tijd
  • Een bruto maandsalaris tussen de € 3.560,- en € 4.716,- o.b.v. 36 uur (cao GGZ, FWG 50)
  • Tegemoetkoming in de reiskosten woon-werkverkeer. Voor dienstreizen ontvang je € 0,50 per kilometer!

Jouw rol als Ambulant Begeleider

Als ambulant begeleider binnen het IHT-team werk je met mensen die zich in een acute psychiatrische crisis bevinden. Je wordt vaak ingeschakeld via de huisarts of ketenpartners wanneer de situatie dreigt te escaleren. Je werkt ambulant: je gaat naar cliënten toe, beoordeelt samen met het team wat nodig is en ondersteunt bij herstel en stabilisatie in de eigen leefomgeving. Het doel is helder: voorkomen dat klachten verergeren en een klinische opname nodig wordt of, als opname toch nodig is, deze zo kort mogelijk houden.

Je werkt intensief samen met het steunsysteem rondom de cliënt (naasten, huisarts, andere zorgverleners). Dagelijks neem je deel aan het multidisciplinair overleg en de IHT-bordbespreking, waarin ieders expertise samenkomt.

Jouw taken op een rij

  • Ambulant begeleiden van cliënten met acute ernstige psychiatrische problematiek
  • Stabiliseren van situaties in de thuissituatie, samen met het multidisciplinaire team
  • Samenwerken met huisarts, netwerk en andere zorgprogramma’s binnen volwassenen- en ouderenzorg
  • Voorkomen of verkorten van klinische opname
  • Dagelijks deelnemen aan multidisciplinair overleg en gezamenlijke besluitvorming
  • Werken volgens de IHT-visie: beter worden doe je thuis.

Het IHT-team is actief in Oss, Helmond en Boxmeer en biedt intensieve thuisbehandeling bij acute psychiatrische problematiek. Het team bestaat uit psychiaters, ambulant begeleiders en sociaal psychiatrisch verpleegkundigen en wordt aangestuurd door een manager behandelzaken. Het IHT Helmond Peelland staat bekend als een professioneel en collegiaal team, waarin samenwerking vanzelfsprekend is en verantwoordelijkheid wordt gedeeld. We leren van elkaar, kijken met elkaar mee en ondersteunen elkaar waar nodig, precies wat nodig is in ambulante crisiszorg.

  • Beter worden doe je thuis, in de eigen omgeving, tussen de mensen die belangrijk zijn
  • Jouw cliënt is mijn cliënt, iedere behandeling is multidisciplinair
  • We doen een beroep op gezonde krachten van cliënt en netwerk
  • Opname zien we als een tijdelijk intermezzo binnen een ambulant traject
  • Cliënten blijven in principe maximaal 6 weken bij IHT in zorg (met uitzonderingen)
  • De 24/7 crisisdienst is tijdens kantoortijden geïntegreerd binnen IHT

Dit breng je mee

  • Een afgeronde mbo- of hbo-opleiding Verpleegkunde (BIG-geregistreerd) of Social Work (profiel Zorg)
  • Bij voorkeur ervaring binnen de ambulante en/of acute GGZ
  • Je werkt zelfstandig, cliëntgericht en handelt professioneel in crisissituaties
  • Bereidheid om eens per 4 tot 6 weken een weekenddienst te draaien (zaterdag én zondag overdag, 6 uur per dag)
