Buscamos Enfermeiro(a) Plantonista para atuar no acompanhamento e condução de pacientes dos programas assistenciais, realizando atendimento domiciliar, procedimentos de enfermagem e ações de cuidado de diferentes níveis de complexidade.Esse profissional terá papel importante na integração entre a CaptaMed, pacientes, familiares, cuidadores, equipe multidisciplinar e operadoras de saúde, contribuindo para a continuidade e a qualidade da assistência.

Responsabilidades e atribuiçõesRealizar visitas domiciliares aos pacientes, conforme solicitação dos convênios e plano de cuidados estabelecido.Avaliar, acompanhar e conduzir os casos dos pacientes atendidos pela CaptaMed.Planejar, executar e acompanhar os atendimentos dos programas assistenciais.Realizar procedimentos de enfermagem de baixa, média e alta complexidade em ambiente domiciliar.Orientar e capacitar familiares, cuidadores e/ou técnicos de enfermagem quanto aos cuidados necessários ao paciente no domicílio.Realizar treinamentos periódicos para cuidadores.Atuar na intermediação entre a CaptaMed e as famílias dos programas assistenciais.Prestar suporte a familiares e convênios na condução dos casos.Participar de reuniões para discussão de casos clínicos, alinhamento da proposta terapêutica e definição do plano de cuidado.Acompanhar indicadores da carteira, incluindo internações, eventos adversos e ocorrências.Realizar gerenciamento de riscos relacionados aos pacientes.Registrar evoluções, intercorrências, eventos adversos e ocorrências no sistema e prontuário.Controlar o estoque de materiais e medicamentos dispensados e registrar os saldos no Sistema IW.Elaborar e encaminhar relatórios de enfermagem conforme os prazos estabelecidos.

Requisitos e qualificaçõesObrigatórios:Graduação completa em Enfermagem.Experiência profissional na área de Enfermagem.Registro profissional regular no Conselho Regional de Enfermagem (COREN).Carteira de habilitação categoria B.

Será considerado diferencial:Experiência com pacientes em estado crítico de saúde.Experiência em serviços de atendimento domiciliar.Conhecimento do Sistema IW (Incoway).
