Responsável pela assistência de enfermagem em ambiente domiciliar e externo, garantindo um atendimento humanizado, seguro e de qualidade ao paciente, de acordo com protocolos assistenciais, prescrição médica e de enfermagem e normas institucionais. Atua no acompanhamento da evolução clínica, planejamento dos cuidados, orientação aos familiares e interface com a equipe multidisciplinar.

Profissional humanizado, responsável e organizado, com capacidade de avaliação clínica, tomada de decisão e trabalho em equipe, comprometido com a segurança do paciente, qualidade da assistência e continuidade do cuidado no ambiente domiciliar.

Responsabilidades e atribuições·         Monitorar o estado clínico do paciente em atendimento domiciliar e externo.

·         Acompanhar o paciente em atividades externas, como fisioterapias e demais atividades.

·         Elaborar, implementar e acompanhar o plano de cuidados de enfermagem.

·         Administrar medicamentos e realizar cuidados com dispositivos, curativos, demais procedimentos pertinentes.

·         Monitorar sinais vitais, evolução clínicas e possíveis intercorrências, adotando as medidas necessárias e comunicando a equipe e os familiares.

·         Realizar registros completos e atualizados da assistência prestada.

·         Identificar riscos assistenciais e implementar medidas preventivas para garantir a segurança do paciente.

·         Atuar na prevenção e controle de infecções relacionadas à assistência à saúde.

·         Participar de discussões e alinhamentos com equipe multidisciplinar, médicos, fisioterapeutas, nutricionistas e demais profissionais envolvidos no cuidado.

·         Acompanhar indicadores assistenciais e contribuir para a melhoria contínua da qualidade do atendimento.

·         Identificar e registrar não conformidades, intercorrências e eventos adversos, contribuindo para a elaboração de ações corretivas e preventivas.

·         Zelar pelo cumprimento dos protocolos, normas técnicas, éticas e de segurança do paciente.

·         Executar outras atividades relacionadas à assistência de enfermagem e ao atendimento domiciliar.

Requisitos e qualificações·         Graduação completa em Enfermagem ou Técnico.

·         Registro ativo no COREN.

·         Experiência em assistência de enfermagem, diferencial em Home Care.

·         Conhecimento em avaliação clínica e elaboração de plano de cuidados.

·         Conhecimento em administração de medicamentos e procedimentos de enfermagem.

·         Conhecimento em segurança do paciente e prevenção de infecções.

·         Boa comunicação, ética e relacionamento interpessoal.

·         Organização, responsabilidade e capacidade de tomada de decisão.
