A OSID – Obras Sociais Irmã Dulce é uma das maiores instituições filantrópicas do país, com cerca de 5,6 milhões de procedimentos ambulatoriais por ano, na Bahia, a usuários do Sistema Único de Saúde (SUS), idosos, pessoas com deficiência e com deformidades craniofaciais, pessoas em situação de rua, usuários de substâncias psicoativas e crianças e adolescentes em situação de risco social. A entidade conta com um perfil de serviços único no Brasil, distribuídos em 21 núcleos que prestam assistência à população de baixa renda nas áreas de Saúde, Assistência Social, Pesquisa Científica, Ensino em Saúde, Educação e na preservação e difusão da história de sua fundadora, Santa Dulce dos Pobres.Responsabilidades e atribuiçõesAtuar na navegação da jornada do paciente oncológico, desde a confirmação diagnóstica até o início do tratamento, promovendo o acolhimento, acompanhamento sistemático, identificação e resolução de barreiras, articulação entre as equipes e monitoramento dos tempos assistenciais, contribuindo para o acesso oportuno e a continuidade do cuidado.Escopo de atividadesRealizar acolhimento e acompanhamento individualizado do paciente oncológico durante sua jornada assistencial.Realizar busca ativa de pacientes elegíveis para navegação e acompanhar sua evolução no processo diagnóstico e terapêutico.Monitorar o fluxo do paciente desde o diagnóstico até o início do primeiro tratamento.Acompanhar sistematicamente o prazo para início do tratamento oncológico, conforme legislação vigente e protocolos institucionais.Identificar pendências assistenciais, administrativas e documentais que possam atrasar o início do tratamento.Atuar na resolução e no acompanhamento das pendências, articulando as áreas responsáveis.Monitorar consultas, exames, procedimentos, avaliações e demais etapas necessárias à definição e início do tratamento.Articular a comunicação entre pacientes, familiares, equipe médica, enfermagem, equipe multiprofissional, regulação e áreas administrativas.Orientar pacientes e familiares quanto aos fluxos, etapas do cuidado, agendamentos e encaminhamentos necessários.Realizar acompanhamento ativo de pacientes com faltas, perda de seguimento ou risco de atraso no tratamento.Registrar no prontuário e/ou sistemas institucionais as ações de navegação, contatos, pendências e encaminhamentos realizados.Monitorar indicadores relacionados à jornada do paciente, tempo de espera, pendências e início do tratamento.Elaborar e atualizar controles e relatórios gerenciais da navegação oncológica.Participar da análise das causas de atrasos e propor ações de melhoria dos fluxos assistenciais.Contribuir para a integração da linha de cuidado oncológico e para a melhoria da experiência do paciente.Apoiar a implantação, organização e melhoria contínua do Programa de Navegação Oncológica.Atuar em conformidade com protocolos assistenciais, normas institucionais, legislação vigente, segurança do paciente, ética profissional e sigilo das informações.Contribuir para o cumprimento das metas e indicadores assistenciais estabelecidos no contrato de gestão com a SESAB.Requisitos e qualificaçõesGraduação em Enfermagem com registro ativo no COREN;Experiência em oncologia ou em processos relacionados à linha de cuidado oncológico.Desejável curso de Navegação;Informações adicionaisPlano de saúde e odontológico;Vale transporte;Almoço no refeitório.
